  • Full name of the attorney in fact (AIF)


  • If the AIF is not a client, we will need to establish them as an authorized persons and will need the following information:

    • Full name

    • DOB

    • SS#

    • Address

    • Phone number

    • Email


  • Do they wish to name a successor AIF? If they would like to name a successor the following is needed of not already a client:

    • Full Name

    • SS#


  • Which accounts are we applying this POA to?

    • All accounts

      • if not all accounts which ones?


  • What type of Authority type is to be granted?

    • Full

    • Limited

    • Information only


  • Do you want your AIF(s) to also be able to name and change beneficiaries?

    • If yes, signatures must be notarized

    • ·Signatures must be notarized if authority to change beneficiaries is granted and the Durable Power of Attorney is governed in California. This will disable Send

      for eSignature and eSign in Person.

    • Signatures must be notarized if the Durable Power of Attorney is governed in California. This will disable Send for eSignature and eSign in Person




Affidavit of Durable Power of Attorney - ** If you are completing this flow, the affidavit must be supported by a non-Ameriprise Power of Attorney Document. It can be uploaded to the case upon completion. **
